How to send baseball card for grading?

I just bought this very nice Derek Jeter Jersey Card. How do I send this card in for "grading". It would be really nice because he will go into the HOF and most likely get 3,000 hits by the end of the season. So I was just wondering what's the fee and company that I send this card too?
